Nomination Package

Each Nomination Package contains one (1) nomination form and two (2) reference forms.

• Nomination Package

Nomination Form - to be completed, and submitted, by the person nominating the student.

The nominating person can be anyone except the candidate or one of the candidate's two references.

Reference Forms - two (2) reference forms must be completed and submitted with each nomination form.

One reference must be a teacher, school counsellor, or school administrator.

The other reference may be a friend, family member, employer, member of an outside agency or community organization, or someone who has been personally touched by the efforts of the candidate to promote pos itive change.

Deadline for application: March 31,2010

Award Announcements: June 1, 2010

Helpful Hints

The application must be submitted as a single package (one nomination form, and two reference forms).

The nominating person is responsible for ensuring that the application is complete.

Incomplete applications will not be considered.
