Nomination Process

1. Read about the criteria and eligibility information.

2. Download and complete the Nomination Form, which includes the consent of a parent/guardian for the nomination of the student candidate.

Complete the single page description included on the nomination form, explaining why the candidate deserves this award.

3. Download two copies of the Reference Form and have each of the two references complete the form and return them to you for inclusion in the nomination package.

Please ensure the references have also completed the one page description included on the reference form.

All nomination forms, reference forms and the one page description of the student’s achievements must be included in one package.

4. Mail or drop off the completed package to the following address by the March 31, 2010 deadline:

Helpful Hints

The application must be submitted as a single package (one nomination form, and two reference forms).

The nominating person is responsible for ensuring that the application is complete.

Incomplete applications will not be considered.
